Chattanooga Intelect Focus Shockwave Set

The Chattanooga Intelect Focus Shockwave Set is a focused extracorporeal shockwave system that uses electromagnetic technology to generate its waves. The handpiece couples to the patient through a water bag on the head, and energy is selected in steps from 0.01 to 0.55 mJ/mm2 at a repetition rate of 1 to 8 Hz, in single shock or continuous shock mode. The Focus is a different machine from the radial Intelect RPW 2 and Intelect Mobile 2 RPW, which are pneumatic units metered in bar, and it shares no parts with either of them in either direction.

Two conditions govern who runs this set, and the first is regulatory: the Intelect Focus is an FDA Class III device whose instructions for use carry a single cleared indication that the operator is expected to work inside. The second is training: Chattanooga requires the operator to have completed a device-specific training course on the Chattanooga Intelect F-SW USA, which is a stricter requirement than either radial Intelect unit carries, and the course is tied to this machine alone. How the Focus Aims and Meters Its Energy

A session on the Intelect Focus is set by the energy step in mJ/mm2 and the repetition rate in Hz, and the stand-off fitted to the handpiece head decides how near the surface that energy lands. Focal volume follows from the energy step you choose, tightening from 0.87 cm3 at 0.10 mJ/mm2 to 0.14 cm3 at 0.55.

Energy is selected in steps across the full range from 0.01 to 0.55 mJ/mm2. Peak compressional pressure reaches 14 MPa at 0.10 mJ/mm2, 36 MPa at 0.35 and 62 MPa at 0.55, with peak rarefactional pressure of 9, 13 and 15 MPa at those same three settings. A radial unit meters its output in bar, so no setting you know from an Intelect RPW 2 carries across to this machine.

The Frequency Ceiling Moves With the Energy Step

The Focus runs in single shock or continuous shock mode at 1 to 8 Hz, and the top of that range is available only at the lowest energy step. The ceiling reads 3 Hz at 0.55 mJ/mm2, 4 Hz at 0.35, 5 Hz at 0.20, 6 Hz at 0.10, 7 Hz at 0.05 and 8 Hz at 0.01, so a session planned at high energy runs at a slower repetition rate and takes longer on the clock.

Stand-Offs Bring the Focal Zone Toward the Surface

With no stand-off fitted the focal zone sits 35 to 65 mm below the coupling surface, and Stand-off I brings it to 15 to 45 mm while Stand-off II brings it to 0 to 30 mm. Therapeutically effective penetration depth at 5 MPa runs 0 to 125 mm, 0 to 105 mm and 0 to 90 mm across those same three configurations, so the stand-off you fit changes the working depth more than any other single choice.

Focal Volume Tightens as the Energy Rises

The minus 6 dB focal zone measures 57 mm axial by 5.4 mm lateral at 0.10 mJ/mm2, 49 by 3.8 mm at 0.35 and 34 by 2.8 mm at 0.55. Focal volume falls from 0.87 cm3 to 0.37 cm3 to 0.14 cm3 across those three settings, so the higher steps concentrate their output into a smaller volume while the lower steps spread it over a longer axis.

The Stand-Off Sets the Depth of the Focal Zone

The handpiece head carries a water bag that couples the shockwave into the patient, and a transparent closing ring retains Stand-off I or Stand-off II on the head. The stand-off is what brings the focal zone toward the surface: 35 to 65 mm with nothing fitted, 15 to 45 mm with Stand-off I, and 0 to 30 mm with Stand-off II. The handpiece weighs 590 g (1.7 lb), carries a service life of 5 million shocks, and takes a coil exchange at 2 million, which makes the coil the first scheduled service item a busy room will meet. Silicone oil and transmission gel are the consumables that go with it, the oil topping up the bag on the head and the gel going on the skin at the coupling face.

The Focus has no pneumatic projectile, no guiding tube and no transmitter thread, so nothing from a radial Intelect RPW 2 or Intelect Mobile 2 RPW fits it: none of the nine shock transmitters, none of the handpieces, none of the revision kits. A clinic that already runs a radial unit budgets for Focus consumables separately, because the two ranges share no wear parts at all and no accessory kit bridges them. Six contraindications apply, and each one is a reason to keep the handpiece away from the area: treatment over or near a bone growth center until bone growth is complete; where a malignant disease is known to be present in or near the treatment area; infection in the area to be treated; a patient with a coagulation disorder or taking anti-coagulant medications; a patient with a prosthetic device in the area to be treated; and over ischemic tissue in individuals with vascular disease effects, for example foot ulcers or venous stasis. One precaution travels with that list: patients may experience pain or discomfort during and after treatment, and treatment with analgesics may be appropriate.

Depth of the focal zone is 35 to 65 mm with no stand-off, 15 to 45 mm with Stand-off I and 0 to 30 mm with Stand-off II. No, and not one of them. The Focus is electromagnetic and focused, with no pneumatic projectile, no guiding tube and no transmitter thread for a transmitter to screw into. The nine D-ACTOR family transmitters, the FALCON handpiece, the R-SW handpiece set, the V-ACTOR sets, both revision kits, the water reservoir and the carry bag all belong to the radial Intelect RPW 2 or Intelect Mobile 2 RPW. The traffic runs the other way too, so no Focus part fits a radial unit either.

Whichever you fit is retained on the handpiece head by the transparent closing ring, so keep a spare ring with the machine, because a damaged ring takes both stand-offs out of service at once.

The water bag, the silicone oil, the transmission gel and the coil. We stock the water bag and a Coil Revision Kit FSW; we do not currently stock the silicone oil or the stand-offs themselves, so source those alongside the machine and keep a spare water bag in the room, since the bag is the part that fails first on a handpiece in daily use.

After 2 million shocks the coil needs to be exchanged. The handpiece itself carries a 5 million shock service life and the control device is rated for 15,000 hours of operation per IEC 60601-1, so in a busy room the coil is the first scheduled service item you will meet. We supply the exchange as the Coil Revision Kit FSW, and it is worth giving us the serial number of your control device when you order so we can confirm the kit against the machine on your floor.

Yes. While applying focused shockwaves at maximum adjustment, do not use more than 6.000 subsequent shocks and stick to a consecutive break of 5 minutes. Working pressure should also be raised slowly to a level of 0.25 mJ/mm2 across the first 500 impulses of a session. Both figures sit in the instructions supplied with the device, and both are worth writing into a room protocol so that every operator runs the machine the same way.
