Chattanooga VitalStim Plus Electrotherapy and sEMG Biofeedback System

The Chattanooga VitalStim Plus Electrotherapy and sEMG Biofeedback System is a handheld swallowing therapy unit that pairs neuromuscular electrical stimulation (NMES) with sEMG biofeedback, which reads muscle activity through surface electrodes and plays it back as a trace on screen and a tone. It is built for clinicians working with patients with dysphagia. Up to four channels stimulate together in VitalStim co-contraction mode. VMS opens phase duration, frequency and work and rest times for protocols of your own. In sEMG plus Triggered Stim, stimulation starts once the patient's own effort reaches a set threshold, adding intensity to the exercise. VitalStim Plus certification training covers the techniques approved for it.

The colour display carries built-in instructional videos on therapeutic exercises and electrode placements, and an anatomical image library covering swallowing anatomy, pathology and treatment imagery. Bluetooth mirrors that screen to a laptop or tablet, where the patient can watch their own muscle activity during the session. Treatment data saves and exports for tracking progress across sessions.

How it works

VitalStim Plus carries two waveforms, one fixed and one adjustable, alongside a two-channel sEMG front end that reads the patient's own muscle activity.

The Fixed VitalStim Waveform
A symmetrical square biphasic waveform with a 100 microsecond interphase interval pulse, fixed at every setting: 0-25 mA constant current, phase duration 300 microseconds at 10 percent accuracy, frequency 80 pps at 5 percent accuracy, cycle time 57 s on and 1 s off, ramp 2 s, treatment time 60 min. In this mode up to four channels deliver stimulation concurrently and pause at the same time. Nothing in the waveform itself is adjustable, so the operator sets intensity and chooses the channels and the rest is fixed.

The Adjustable VMS Waveform
Symmetrical biphasic with a 100 microsecond interphase interval, 0-25 mA constant current, phase duration 60 to 300 microseconds, frequency 1-80 pps, user-defined on and off times of 1-99 s each, ramp 0-3 s and treatment time 1-240 min. It is a low skin load waveform, intended for applications requiring high intensities. That 1-240 min window is four times the fixed VitalStim treatment time, so VMS is the mode with room to build a protocol in.

The sEMG Front End
The sEMG side shows electrical muscle signals on screen with amplified audio feedback, across a range of 0.2 to 2000 microvolt RMS continuous, sensitivity 0.1 microvolt RMS, accuracy 6 percent of the microvolt reading plus or minus 0.3 microvolt. Two of the four modes use it: sEMG on its own, and sEMG with triggered stimulation, where the unit switches to stimulation once the patient's own muscle activity crosses a set threshold. The reference EMG lead wire ships with the system while the reference EMG electrode is a separate order.

Where the Treatment Data Goes
Up to six user-defined protocols can be stored in the VMS and sEMG plus VMS modalities. A micro SD card slot holds saved treatment summaries, and therapy information exports to a PC for the clinical file. Bluetooth mirrors the colour display onto a computer or tablet through a downloadable PC application, with Bluetooth switched on by default in the settings. A stored protocol carries the parameters you set, so a repeat session starts from the same numbers.

Modes

The Patient's Own Effort Triggers the Fourth Mode

VitalStim Treatment runs the fixed waveform in co-contraction, with up to four channels stimulating concurrently and pausing together. VMS Treatment opens the parameters: phase duration, frequency, and work and rest times. sEMG Treatment reads muscle activity and shows it on the colour screen, with amplified audio feedback. sEMG plus Triggered Stim waits for the patient's own effort to cross a set threshold and then switches the unit over to stimulation. Two of the four modes therefore respond to what the patient does, and two are set entirely at the operator's hand.

The Handheld and Its Stand

The VitalStim Plus is a 6.2 in. handheld with a colour display worked by stylus, and it stands upright on its own stand through a session. A hand switch, a rubber sleeve and a spare battery door come in the carton, and four AA cells behind that door power the whole unit. Treatment summaries save to a micro SD card, ready to export to a PC.

One consumable attaches to this system: the VitalStim electrode, built to transmit the high current load and current density of the VitalStim Therapy protocol, a much higher load on the electrode than a typical NMES application in physical medicine and rehabilitation. No. VitalStim electrodes are an optional accessory, sold as VitalStim Plus electrodes: adult in packs of 12, 30, 50 and 100, and small in a pack of 12. The reference EMG electrode for the sEMG side is a separate order too.

Yes. VitalStim Plus certification training covers the accessories and techniques approved for the system, and use of other accessories and/or techniques not approved under the VitalStim Plus certification training may result in death, injury, or adverse effects. The certification course is arranged with Chattanooga.

The unit, snap lead wires, the reference EMG lead wire, a hand switch, a stand, a stylus, a rubber sleeve and a battery door. Electrodes and the reference EMG electrode are sold separately.

The device should NOT be used where there are cancerous lesions in the treatment area; over swollen, infected or inflamed areas or skin eruptions; in patients with demand-style cardiac pacemakers or other implanted electronic devices; over the carotid sinus, particularly with known sensitivity; in severely demented patients with continuous verbalisation; with significant reflux and feeding-tube dependence; in drug-toxicity-induced dysphagia; with undiagnosed syndromes; with serious infectious disease; and in pregnancy, where safety has not been established. Read the manual in full before the first treatment.

Stimulation should not be applied transcerebrally or over the carotid sinus. Caution is advised for patients with suspected or diagnosed epilepsy; electronic monitoring equipment may not operate properly during stimulation. Long term effects of chronic electrical stimulation are unknown. The unit must not be operated in the vicinity of therapeutic microwave or RF shortwave diathermy, and it must not be dropped on hard surfaces or submerged in water.

The VitalStim waveform is fixed and the VMS waveform is adjustable. VitalStim runs a 300 microsecond phase at 80 pps on a 57 s on, 1 s off cycle for 60 min, across up to four channels in co-contraction. VMS lets you set 60 to 300 microseconds, 1 to 80 pps, work and rest times of 1 to 99 s and a treatment time up to 240 min.
